Primarily for his attitude: Notice on Leg 2, he veered way outside of the base leg turn, he got dangerously close to an aircraft landing on 10L. and that aircraft mentioned it. I did the research on this, he overshot his base leg turn by 604 feet. the span between 10R and 10L is 1470, so he was almost half way into 10L's corridor. The Pilot, a ATP/CFI/FE Rated pilot of more than 20 years, has quite a history that will make you go "WTF" This guy definitely needs to lose his CFI Status at least.
Agreed. I watched and commented the video on YT, some people disagreeing that the controller should have simply told him to write down a number to call and fill out an incident form. With these numbers, this is definitely "possible pilot deviation" and should be investigated. Better get him to lose his license this way instead of burning it, along with him and an unfortunate student after a crash.
